Prior to Starting, You Will Need:
Insurance cards (if applicable). YOUR APPOINTMENT REQUEST WILL BE DECLINED IF INSURANCE INFORMATION IS NOT INCLUDED UNLESS YOU ARE SELF PAY. If you need a referral please make sure that is requested from your PCP before booking your appointment.
Access to your email (for security purposes, you will receive an email with a 4-digit security code to enter during scheduling).

Appointment Types: Skin cancer screenings (Full Body Skin Exam), acne, rashes and hair loss require detailed, focused evaluations. A dedicated visit is required to thoroughly address these complaints. For example, if you have concerns about both hair loss and would like a skin cancer screening, please book two separate appointments -or- book your most concerning issue and we can schedule your subsequent visits in office. Cosmetic Consultations are reserved for those who are seeking to treat issues not covered by insurance such as wrinkles and/or volume depletion with Neuromodulators (such as Botox and Dysport) or Filler ONLY. Other issues such as pigmentation or suspicious lesions need medical evaluation prior to treatment, which could include cosmetic options.
